I would also like the search list to be visible to friends and family, they always ask for Christmas lists or birthday gift tips and when I send them an excel sheet (via export) they don't understand it or they can't even open it.

Well I like the idea with the tick otherwise better, because if you are going to work with a friends list then all the people you want to show it to should create an account before you can add them as a friend. If I have to explain that to their parents or something, they also say 'never mind'. if you implement something like this, you can set the search list to not visible by default until you tick the check mark in your profile options

Opening it up to everyone is indeed not a good idea, but for example, putting a question (of your own choice) as access to your list works fine (see alsolijstje.nl). Such a question could be, for example, "What sport likes best", "What is my son's name" or "What is my wife's maiden name".

Why would you only want to limit this to your "friends". Why not just leave the responsibility with each individual user whether or not they want to open their search list. You can also make an option: "only" for friends.

O only visible to friends

O Visible to everyone

You could also add the option that only parts or individual items of a search list can be made visible by checking the box.

The two options: visible or invisible are sufficient. The other friend option requires more customization because CW does not have a friend feature. I suggest not waiting for that friends feature.

I have a number of items that I would love if people search with me, but I have the vast majority on my wish list because I am interested in them but don't really want to buy them yet.
